The Clark County Combined Health District (CCCHD) will present the findings of its 2015 Community Health Assessment on Jan. 29 and will begin collaborating with community partners to create the 2016 Community Health Improvement Plan.

The CCCHD, Springfield Regional Medical Center, Mental Health Recovery Board and the Rocking Horse Community Health Center invite you to join us as we begin the discussion and planning for the improvement plan. Our goal is to use it as a road map for all health agencies for the coming years.
